Family Support Program
The Family Support Program offers planning and support
services for developmentally challenged children and their families
in Middlesex County.
Population Served
Developmentally challenged children 18 years of age
and under and their families who live in Middlesex County.
Available Services
- Comprehensive information and referral to appropriate community
service for children with a developmental disability and their
families.
- Case Management –- Staff will assist families to develop
support plans of their choosing.
- Advocacy –- Staff will advocate with or on behalf of
people having difficulty accessing service.
- Crisis –- staff are available to become involved in situations
where there has been a drastic change in support needs.
- Community Development/Education –- Staff are available
to assist families to link with natural community supports.
- Life and Future Planning –- Staff will facilitate planning
towards a written plan that can be used for pursuit of resources.
